Please review the following and all statements before submitting the 3 forms. All 3 forms must be completed to be considered for ​the team.

Before submitting payment, please provide all required details regarding your student-athlete. By checking the box below, you confirm that you have read and agree to the stated terms and conditions.

Terms: 
1. Only 3 practices can be missed in a season. (Illness, funeral, travel plans are excused with prior notice)
2. Student-Athlete must maintain a B average to remain on the team.
3. Qualifying meets must be paid for before the meet. (Will only pay for the meets your athlete is qualified for.)

All Paid Services are Final.

Media Release

Please read this statement before completing the form.

I, the parent/guardian of the athlete named on the form, grant permission to Frisco Phoenix Track Club, its representatives, coaches, volunteers, and authorized media partners to photograph, record, and/or videotape the participant during practices, meets, events, and team-related activities.

These images, audio recordings, or videos may be used for legitimate purposes, including, but not limited to:

  • Team websites and social media platforms
  • Promotional materials (flyers, posters, programs)
  • News media coverage
  • Team presentations or highlight videos

I understand that these materials may be used without compensation and that all content becomes the property of Frisco Phoenix Track Club.

Travel Authorization Form

Please read and complete the form for your athlete.

I understand that the Frisco Phoenix Track Club may provide transportation to and from practices, meets, and team-related events under the supervision of authorized coaches and team staff.
By signing this authorization, I grant permission for the Frisco Phoenix Track Club coaches and designated staff to transport my child to and from track practices, competitions, and related team activities, whether by team-arranged transportation or approved personal vehicles, as permitted by team policy.

I acknowledge and agree that:
  • Coaches and staff are permitted to assume temporary supervision of my child during travel to and from team events.
  • This authorization applies only to official team activities.
  • I am responsible for informing the coaching staff in writing of any changes or restrictions to this authorization.
Without a signed Travel Authorization on file, I understand that:
  • I, or another approved parent/guardian, must be present for drop-off and pick-up of my child at all practices and events.
  • My child will not be permitted to ride with coaches or team-arranged transportation to or from any event.
